Insurance Note (Please Review):
We are actively working to transfer insurance credentialing to the new location. We expect to be able to see patients with BCBS, Providence, Aetna, and OHP right away; however, re-credentialing with other insurance plans may take longer. During this transition, some visits may need to be self-pay.

Insurance credentialing timelines are outside of our control and can take several months to complete.

Returning patients are eligible for 20% off the standard cash rate during this transition period.
