All intake forms and consents must be completed prior to your first visit. Completing these in advance allows your appointment time to focus fully on your story, goals, and care plan.

If you are transitioning from Dr. Turner’s previous practice location, you will also receive:

An Authorization to Release Confidential Information

The authorization to release confidential information form allows us to request your prior chart notes so your care can continue seamlessly. We ask that all forms be completed before your first visit to avoid delays.
