Appointment Types
FREE Intro Consultation (15 min)
A 15-minute call to connect, ask questions, and make sure this feels like a good fit.
Follow-Up Visit (Adult - 45-60 min)
For ongoing care, physical exams, PAPs, pelvic floor therapy, abdominal & uterine massage, and other hands-on treatments.
Follow-Up (Telemedicine 45 min)
For ongoing care for adults and pediatric patients. Best for lab review, medication management, nutrition and lifestyle counseling, and care plan updates.
New Patient Visit (60 min)
For first-time patients. Includes full intake, history, and initial treatment plan. Please select Adult or Pediatric.
​Follow-Up Visit (Pediatric 45-60 min)
For ongoing care for infants, children, and adolescents, including wellness visits, immune support, and developmental concerns.
Medication Management (Telemedicine)
Shorter follow-up appointment for patients needing prescription refills, dosage adjustments, or support with medication-related symptoms. ​​
Scheduling Policies
We use secure messaging and text communication to keep things simple and connected, including reminders, forms, and care updates. You’ll also need an email address to access your patient portal and complete intake paperwork.

We do require a credit card on file for billing purposes, with the exception of those covered by Medicaid.

All intake forms and consents must be completed prior to your first visit. Completing these in advance allows your appointment time to focus fully on your story, goals, and care plan.
Need to Cancel?
Please call +1 (971) 366-3308 or send a message on your ChARM patient portal.
We ask for at least 48 hours’ notice if you need to cancel or reschedule. Appointments canceled with less notice, or missed without notice, may be subject to a $75 late cancellation fee. This fee is not covered by insurance. This policy helps protect appointment availability and supports the sustainability of a small practice.

Please note the following upcoming office closures. During these dates, the clinic will be closed and I will be unavailable for appointments, patient portal messages, prescription refills, paperwork requests, and other non-urgent patient communication.
