Become a New Patient

We are currently accepting new patients (28 days and older) and would be honored to support your care.

Scheduling a New Patient Visit below is the first step in getting started. If you’re scheduling for a child or dependent, please use their name and date of birth along with the guardian’s contact information.

New patient appointments are requests only and are not confirmed right away. We’ll follow up within three business days to confirm availability and next steps.

Not sure where to Start?

Schedule a 15-minute Intro Consultation. This is a chance to briefly connect, ask questions, and make sure this feels like a good fit. The cost is $45 and is not billable to insurance. 

If you decide to move forward and schedule a New Patient Visit at the time of your call, the cost may be applied toward your first visit at the equity-tier cash rate upon request. 

Appointment Types

New Patient Visit (60 min)

For first-time patients. Includes full intake, history, and initial treatment plan. Please select Adult or Pediatric. 

Follow-Up Visit (Pediatric)

For ongoing care for infants, children, and adolescents, including wellness visits, immune support, and developmental concerns.

Follow-Up Visit (Adult)

For ongoing care, physical exams, PAPs, pelvic floor therapy, abdominal & uterine massage, and other hands-on treatments.

Follow-Up (Telemedicine)

For ongoing care for adults and pediatric patients. Best for lab review, medication management, nutrition and lifestyle counseling, and care plan updates.

Scheduling Policies

We use secure messaging and text communication to keep things simple and connected, including reminders, forms, and care updates. You’ll also need an email address to access your patient portal and complete intake paperwork.

We do require a credit card on file for billing purposes, with the exception of those covered by Medicaid.

All intake forms and consents must be completed prior to your first visit. Completing these in advance allows your appointment time to focus fully on your story, goals, and care plan.

Need to Cancel?

 Please call 503-841-2630 or send a message on your ChARM patient portal.


We ask for at least 48 hours’ notice if you need to cancel or reschedule. Appointments canceled with less notice, or missed without notice, may be subject to a $75 late cancellation fee. This fee is not covered by insurance. This policy helps protect appointment availability and supports the sustainability of a small practice.
