Community Supported Direct Care
Community Supported Direct Care is a direct-pay healthcare model with transparent pricing and no insurance middleman. You pay for visits directly, and pricing includes equity tiers so that those with greater financial stability help sustain access to care for others in the community. There is no membership or subscription fee at this time. You pay per visit.
Everyone. This model is grounded in the belief that community supporting community strengthens healthcare for all. It is designed to sustain thoughtful, relationship-centered care while expanding access. We thrive when we support one another.
Special intention is given to serving those impacted by systemic inequity, including people who identify as Black, Indigenous, or People of Color; disabled or chronically ill; LGBTQIA+; larger-bodied; low-income; or who hold multiple marginalized identities.

The tiered pricing model was developed to balance two priorities: keeping the clinic financially sustainable while expanding access to care. The standard price point reflects what is needed to support the practice and provide high-quality care, while still remaining below typical market rates for specialty care visits.
The tier structure allows patients who are able to contribute more to help support reduced-cost access for others.
For patients who anticipate needing more frequent visits (such as weekly pelvic floor therapy appointments) or who are navigating financial constraints, additional discounts can always be discussed directly with the provider.
The goal is to keep pricing transparent, flexible, and rooted in community.

If specialty care is needed, we'll help connect you with the right provider and coordinate care so communication stays clear and collaborative. Any fees for specialist visits are set by that office and depend on your insurance plan or their cash-pay structure.
When testing is needed, we've got your back! If you’re using insurance, labs and imaging are billed directly through the facility performing the test according to your plan benefits. If you’re uninsured, do not have lab benefits, have a high deductible, or prefer not to use insurance, we can arrange discounted cash-pay options instead. We've secured reduced pricing for many common labs and imaging studies, and we review costs beforehand so you know what to expect.
If a prescription is part of your care plan, Dr. Turner can write it for you as long as you are an established patient in the practice. You may use your insurance at the pharmacy of your choice, or pay out of pocket if you prefer. When helpful, we can also provide GoodRx discount options to help lower medication costs.
Vaccination decisions are personal and often complex. Our goal is to provide safe, non-judgmental care and to offer quality, evidence-informed information so families can make the best decision for their child and their family. We will never turn anyone away based on their choice to vaccinate or not. Your family’s health and well-being are our top priority.
We do not currently offer vaccines at our clinic, but we will provide reliable referrals to help meet your family’s preventive healthcare goals. Families who choose to vaccinate are encouraged to maintain a relationship with a primary pediatric provider or local health department for immunizations and required documentation.
In most cases, yes. Visits are typically eligible expenses for Health Savings Accounts (HSA) and Flexible Spending Accounts (FSA). We can provide detailed receipts or documentation if needed. Because plans vary, we recommend confirming specific eligibility requirements with your HSA or FSA administrator.
For most people, it is still wise to maintain health insurance for major medical events such as emergencies, hospitalizations, surgeries, advanced imaging, or specialist care. These types of services can be very expensive and are outside the scope of what our clinic can provide.
That said, many of your routine primary care needs can be addressed directly within this practice through transparent visit pricing and direct-pay services.
Because of this, some patients choose to carry high-deductible or catastrophic insurance plans to protect against large unexpected medical expenses while using direct care for their day-to-day healthcare.

Community-Supported Direct Care is related to Direct Primary Care, but it is structured differently to improve accessibility.
Many Direct Primary Care practices use a monthly membership model and may require multi-month or annual contracts. While this works well for some people, those types of commitments can be financially or logistically inaccessible for many patients.
Instead, this practice uses a direct fee-for-service model, meaning patients pay for visits when they need them rather than committing to a monthly membership. This keeps care flexible and accessible while maintaining transparent pricing and longer, unhurried visits.

Due to ongoing issues with non-payment and significant under-reimbursement, we have made the decision to discontinue participation with the following plans:

Access to care through the Oregon Health Plan matters deeply to our practice. OHP is a critical safety net that allows many people to receive care they would otherwise be unable to access, and we believe strongly that income, disability status, family structure, or life circumstance should never be a barrier to receiving compassionate, high quality healthcare.
We are committed to always accepting OHP as part of our practice because equitable access to care is a core value of our work.
You are welcome here, even if you don’t have insurance. While we recommend maintaining coverage for emergencies or hospital care, insurance is not required to work with us. This practice operates on a Community Supported Direct Care model with transparent, equity-based pricing.
We offer discounted cash-pay rates for labs and imaging, and if you need specialty care, we will help you find options that are both clinically appropriate and financially manageable.
Yes, we welcome Medicare patients! However, we do not bill medicare insurance for visits. Appointments will be paid at the time of visit using the Community Supported Direct Care model. Learn more here.
Medicare can still be used for most labs, imaging, prescriptions, and specialist care ordered outside the practice.
We want to be transparent about why we are intentionally moving toward a community supported direct care model.
In Oregon, naturopathic physicians do not have pay equity with other primary care providers, including MDs, DOs, NPs, and PAs. For the same visit types and billing codes, reimbursement is often lower, inconsistent, or denied altogether. Insurance companies may require prior authorizations simply to be seen by a naturopath, exclude licensed services, or retroactively deny care.
We have patients who pay more in monthly insurance premiums than we receive in an entire year of care from their plan.
These are not isolated issues. They reflect a healthcare system that prioritizes profit and bureaucracy over care. In the current political and economic climate, continued reliance on this model harms patients, practitioners, and clinic staff. It pushes healthcare into a transactional framework where time, relationship, and healing are devalued.
Our long-term vision is to move away from transactional healthcare models and toward systems where everyone involved is supported. This means patients receiving care without fear of denials or surprise bills, practitioners being fairly compensated for their labor, and staff working within sustainable and humane structures.
As part of this vision, we are committed to always accepting OHP. Access to care matters deeply, and this commitment exists alongside our broader goal of building a transparent, community-supported, equity-based practice that allows all of us to be cared for.
The amount you pay depends on your specific insurance plan, and it is not always as straightforward as it may seem.
Most patients are responsible for some combination of:
A copay, which is a fixed fee due at the time of your visit
A deductible, which must be met before your insurance begins to cover services
Any remaining balance for services that are not fully covered
Not always. A copay is simply the amount collected at the time of your visit, but it may not reflect the total cost of care.
Depending on your plan, you may still receive a bill if:
Your deductible has not been met
Additional services are provided
Your insurance does not fully cover certain aspects of your care
Coverage varies by plan, but commonly uncovered or partially covered services can include:
Extended or complex visits
Non-covered services (Examples may include: bodywork, pelvic floor therapy)
Lab interpretation and review
Care coordination and communication outside of your visit
Administrative work such as prior authorizations
These are all important parts of your care, but insurance does not always cover them fully.
This is usually related to how your insurance processes the claim.
If your deductible has not been met or if certain services are not covered, the remaining balance is assigned to you by your insurance plan.
Insurance plans can be complex, and it is not always possible to know exact costs in advance.
